  Macrophage activation syndrome (MAS) is a severe complication of rheumatic diseases in childhood, mainly systemic Juvenile Idiopathic Arthritis (sJIA). MAS is typically characterized by high fever, lymphadenopathy, hepatosplenomegaly, hemorrhages, and signs of liver, central nervous system, and kidney involvement, potentially leading to multiple organ failure. Laboratory abnormalities in MAS include leukopenia, thrombocytopenia, anemia, hypertransaminasemia, markedly elevated ferritin levels, and evidence of intravascular coagulation activation. MAS is generally classified among the secondary forms of hemophagocytic lymphohistiocytosis (sHLH), as it occurs in the context of rheumatic disease. MAS and HLH share numerous clinical and laboratory features, with notable differences attributable to the inflammatory background of MAS, such as higher platelet counts and fibrinogen levels compared to primary HLH (pHLH).
