  Pulmonary sarcoidosis is a complex multisystem disorder with an elusive origin, predominantly affecting young and middle-aged adults and characterized by lung involvement. Approximately 95% of individuals with sarcoidosis will experience some form of pulmonary disease at some point in their lives. The progression of sarcoidosis within the lungs is highly variable, encompassing a spectrum that ranges from spontaneous resolution to advanced fibrocystic disease. While the majority of cases follow a more benign clinical course with a propensity for spontaneous resolution, about 5% of individuals face the development of permanent severe pulmonary dysfunction. This subset of patients contributes significantly to the morbidity and mortality associated with sarcoidosis, with respiratory failure being the leading cause of death in the USA and Europe.
