Macrophage activation syndrome (MAS) is a severe complication of rheumatic diseases in childhood, mainly systemic Juvenile Idiopathic Arthritis (sJIA). MAS is typically characterized by high fever, lymphadenopathy, hepatosplenomegaly, hemorrhages, and signs of liver, central nervous system, and kidney involvement, potentially leading to multiple organ failure. Laboratory abnormalities in MAS include leukopenia, thrombocytopenia, anemia, hypertransaminasemia, markedly elevated ferritin levels, and evidence of intravascular coagulation activation. MAS is generally classified among the secondary forms of hemophagocytic lymphohistiocytosis (sHLH), as it occurs in the context of rheumatic disease. MAS and HLH share numerous clinical and laboratory features, with notable differences attributable to the inflammatory background of MAS, such as higher platelet counts and fibrinogen levels compared to primary HLH (pHLH). Primary HLH is caused by mutations in genes encoding proteins involved in perforin-mediated cytolytic function, including perforin and proteins associated with vesicle transport and membrane fusion. Secondary or acquired HLH, which appears more frequent than pHLH, can be triggered by various conditions. Among infection-associated HLH, Epstein-Barr Virus is the most common trigger, with other typical triggers including Herpes viruses, Leishmania, and H1N1 influenza. HLH can also be associated with lymphoma and leukemia, particularly T-cell receptor gamma-delta bearing T-cell lymphomas, which are more common in adults than in children. Additionally, HLH can arise in the context of an underlying rheumatic disease, with sJIA being the most frequent association, referred to as MAS or rheuma-HLH by pediatric rheumatologists. In some cases, no apparent underlying disease or infection is identifiable. Early treatment should include corticosteroids, often in conjunction with intravenous gamma globulin. This approach usually leads to rapid symptom resolution. Steroid doses should be tapered gradually to prevent relapse. If initial treatment fails to significantly improve the patient's condition, then treatment with HLH-directed regimens should be initiated.
