PRKAG2 syndrome is a rare, autosomal
dominant glycogen storage cardiomyopathy caused by PRKAG2 mutations.
This disrupts the AMP-activated protein kinase (AMPK) energy sensor, driving
massive, toxic glycogen accumulation within cardiomyocytes and the cardiac
conduction system. Patients present with a hallmark clinical triad: severe
cardiac hypertrophy (mimicking sarcomeric hypertrophic cardiomyopathy),
ventricular pre-excitation (Wolff-Parkinson-White pattern), and progressive
conduction disease (advanced AV block, sinus node dysfunction). Diagnosis
requires high clinical suspicion when pre-excitation coincides with extreme
hypertrophy, confirmed by definitive genetic sequencing to distinguish it from
sarcomeric HCM or other metabolic phenocopies like Danon or Fabry disease.
Management is highly device-focused as no disease-modifying therapies currently

recurrence rates due to the diffuse infiltrative substrate. Progressive
