cell leukemia (HCL) is a rare, indolent B-cell lymphoproliferative malignancy
driven by the BRAF V600E somatic mutation, which is present in virtually all
classic cases and produces clonal mature B-lymphocyte infiltration of the bone
marrow, spleen, and liver. Patients present with symptomatic splenomegaly,
pancytopenia, characteristic monocytopenia, and recurrent infections.
Immunophenotyping confirming CD19, CD20, CD11c, CD25, CD103, and CD123
co-expression alongside BRAF V600E molecular testing establishes the diagnosis
and distinguishes classic HCL from the variant form. Cladribine or pentostatin
purine analogue therapy delivers high complete remission rates and durable
disease control following a single treatment course, representing the therapeutic
cornerstone. Rituximab combined with purine analogues deepens remission quality
in frontline and relapsed settings. Vemurafenib with or without rituximab
addresses BRAF V600E-positive relapsed disease, while ibrutinib provides an
accessible salvage option in multiply relapsed cases. Prognosis is excellent;
regular haematological surveillance and long-term specialist follow-up are
integral to patient-centred management.
