is an aggressive, highly lethal variant of prostate cancer characterized by a
transition from luminal adenocarcinoma to a neuroendocrine phenotype. While it

Diagnosis requires a tissue biopsy demonstrating neuroendocrine morphology and
immunohistochemical positivity for markers such as chromogranin A,
synaptophysin, and CD56. Because NEPC downregulates AR expression, conventional
androgen deprivation therapy is ineffective. Standard treatment relies on platinum-based
doublet chemotherapy, such as etoposide paired with cisplatin or carboplatin,
mirroring small cell lung cancer regimens. Targeted strategies, including the
delta-like ligand 3 (DLL3) targeted bispecific T-cell engager tarlatamab,
