modalities options for eight major markets (USA, Germany, France, Italy, Spain,
UK, Japan, and China).
Muscle-Invasive
Bladder Cancer (MIBC) Overview
Muscle-invasive bladder cancer (MIBC)
is an aggressive malignancy characterized by the infiltration of urothelial
carcinoma into the detrusor muscle layer of the bladder wall, carrying a high
risk of systemic micrometastasis. Clinically, patients most frequently present
with painless gross hematuria, irritative voiding symptoms, or pelvic pain.
Diagnosis is established via transurethral resection of bladder tumor (TURBT)
to confirm muscular invasion, with staging completed through CT or MRI of the
abdomen and pelvis alongside chest imaging to rule out distant metastases.
Molecularly, MIBC is heterogeneous, categorized primarily into basal-like and
luminal-like subtypes that exhibit distinct clinical behaviors and therapeutic
sensitivities. The definitive treatment standard for localized disease is
neoadjuvant cisplatin-based chemotherapy followed by radical cystectomy with
bilateral pelvic lymphadenectomy. For patients ineligible for cisplatin, or
those with residual disease post-surgery, adjuvant immunotherapy with nivolumab
is standard. Alternatively, bladder-preserving trimodality therapy combining
maximal TURBT with concurrent chemoradiation serves as a highly effective
option for selected candidates. For metastatic or locally advanced disease,
frontline management utilizes the combination of the antibody-drug conjugate
enfortumab vedotin with the PD-1 inhibitor pembrolizumab, a regimen that has
revolutionized survival outcomes over historical platinum doublets.
