(CMT2C) is a rare autosomal dominant axonal peripheral neuropathy caused by
gain-of-function mutations in the TRPV4 gene, encoding a calcium-permeable
mechanosensitive cation channel, resulting in aberrant calcium influx, motor
neuron toxicity, and progressive axonal degeneration. Nerve conduction studies
confirm axonal pathology with reduced compound muscle action potential
amplitudes and preserved conduction velocities. Patients present with distal
limb weakness, foot deformity, sensory loss, and areflexia, with distinctive
additional features including vocal cord paresis causing hoarseness and
diaphragmatic weakness necessitating respiratory surveillance, distinguishing
CMT2C from other axonal subtypes. Diagnosis is confirmed by TRPV4 mutation
identification on hereditary neuropathy gene panel sequencing. No
