Head and neck squamous cell carcinoma
(HNSCC) is a biologically heterogeneous group of malignancies arising from the
mucosal epithelium of the oral cavity, pharynx, and larynx, representing the
seventh most prevalent cancer globally. Two distinct aetiological pathways
drive disease: cumulative tobacco and alcohol carcinogen exposure, and
high-risk human papillomavirus infection. HPV-positive oropharyngeal carcinoma
constitutes a clinically distinct entity, exhibiting profound radiosensitivity
and markedly superior prognosis despite frequently presenting with bulky nodal
metastases. Most patients present with locally advanced disease, requiring
complex function-preserving multidisciplinary management incorporating surgical
resection and concurrent platinum-based chemoradiation. Approximately half of
all patients ultimately experience locoregional recurrence or distant
metastasis, defining the recurrent and metastatic population. Pembrolizumab,
established through the landmark KEYNOTE-048 trial, is the cornerstone of
frontline recurrent and metastatic HNSCC management; monotherapy is standard
for high PD-L1 expressors with combined positive score of 1 or above, while
pembrolizumab combined with platinum-fluorouracil chemotherapy addresses lower
expressors. CPS biomarker assessment is mandatory at diagnosis to guide
therapeutic stratification. Cetuximab-based regimens retain utility in
platinum-ineligible patients. Fianlimab, a LAG-3 inhibitor in combination with
cemiplimab, represents a compelling emerging therapeutic advance demonstrating
significant Phase III activity in recurrent and metastatic disease. Nivolumab
addresses platinum-refractory second-line disease. Prognosis varies
considerably with HPV status, PD-L1 expression, and disease extent;
multidisciplinary tumour board evaluation, functional rehabilitation, and
patient-centred quality-of-life preservation are integral to comprehensive care.
