white matter disease, scientifically classified as leukoencephalopathy with
vanishing white matter or childhood ataxia with central hypomyelination, is a
progressive autosomal recessive leukodystrophy resulting from mutations in any
of the five subunits of the eukaryotic translation initiation factor eIF2B.
This genetic defect impairs the cells’ ability to regulate protein synthesis
during the integrated stress response, leading to the selective vulnerability
and eventual cystic degeneration of glial cells. The pathology is unique
because the cerebral white matter undergoes a transformation where it is
gradually replaced by fluid that matches the signal of cerebrospinal fluid on
neuroimaging. While the condition is characterized by chronic neurological
decline, including ataxia and spasticity, it is punctuated by episodes of rapid
