Membranous Nephropathy (MN) is a
leading cause of nephrotic syndrome in adults, characterized by severe
proteinuria (>3.5 g/day), peripheral edema, and a highly elevated risk of
thromboembolic events. The pathophysiology is driven by the subepithelial
deposition of antigen-antibody complexes—predominantly IgG4 and the C5b-9
membrane attack complex (MAC)—which directly damage podocytes and the
glomerular basement membrane. While historically divided into primary
(idiopathic) and secondary forms, modern nephrology classifies MN by its
specific target autoantigens. The vast majority of primary cases are driven by
autoantibodies against the phospholipase A2 receptor (PLA2R, 70–80%), followed
by NELL-1 (10–20%, which warrants malignancy screening) and THSD7A (1–5%).
Secondary MN is triggered by systemic factors such as infections, neoplasms, or
drugs, with specific biomarkers like EXT1/EXT2 strongly indicating an
underlying autoimmune etiology like systemic lupus erythematosus.
