Unresectable hepatocellular carcinoma
(uHCC) represents an advanced stage of primary liver malignancy in which
curative surgical resection is not possible due to extensive intrahepatic tumor
burden, multifocality, macrovascular invasion, unfavorable tumor location, or
inadequate hepatic reserve in the setting of underlying cirrhosis. The disease
typically arises from chronic hepatic injury associated with hepatitis B or C
infection, alcohol-related liver disease, or metabolic dysfunction–associated
steatotic liver disease, where sustained inflammation, fibrosis, and genomic
instability drive malignant transformation of hepatocytes. Unresectable HCC is
characterized by aggressive tumor biology, prominent angiogenesis, immune
escape, and activation of oncogenic signaling pathways, often accompanied by
progressive deterioration of liver function. Clinically, patients may present
with constitutional symptoms, abdominal pain, ascites, jaundice, or portal
hypertension–related complications, although early manifestations can be subtle.
