Stable (MSS) metastatic colorectal cancer (mCRC) represents approximately 95%
of all mCRC cases and is characterized by a lack of defects in the DNA mismatch
repair (MMR) system. Unlike Microsatellite Instability-High (MSI-H) tumors, MSS
tumors have a lower mutational burden and are considered immunologically
“cold,” meaning they generally do not respond to single-agent immune checkpoint
inhibitors. The pathogenesis involves a chromosomal instability pathway, often
featuring mutations in APC, TP53, and KRAS.
Management is heavily dictated by molecular profiling—specifically the status
