Syndrome Overview
Hepatorenal syndrome (HRS) is a severe
complication of advanced liver disease, characterized by functional kidney
failure in the absence of intrinsic renal pathology, most often occurring in
patients with cirrhosis and portal hypertension, including those with alcoholic
hepatitis or other causes of decompensated liver disease. It is precipitated by
circulatory changes that lead to renal vasoconstriction, and common triggers
include spontaneous bacterial peritonitis, gastrointestinal bleeding, and large‑volume
paracentesis without plasma expansion, while sepsis can also provoke HRS
through vasoactive mediator release. Clinically, HRS is classified into two
types: Type 1, marked by rapid and severe renal decline with a median survival
of less than two weeks, and Type 2, which progresses more slowly and is often
associated with refractory ascites, with survival typically ranging from
several months depending on liver disease severity. Diagnosis requires
exclusion of other causes of kidney injury, and while vasoconstrictor therapy
with albumin may provide temporary improvement, liver transplantation remains
the only definitive treatment offering long‑term survival.
