X-linked
protoporphyria (XLP) is a rare, inherited erythropoietic porphyria caused by a
gain-of-function mutation in the X-linked ALAS2 gene, which leads to the
overproduction and excessive accumulation of the porphyrin precursor,
protoporphyrin IX (PPIX), primarily within red blood cells. The defining
clinical feature is severe, painful, non-blistering photosensitivity upon sun
exposure, typically beginning in childhood, which compels patients toward
strict sun avoidance. While chronic exposure can lead to thickened skin and
scarring, the most serious complication is the risk of hepatobiliary disease,
including gallstones and progressive liver failure in a subset of patients due
to PPIX deposition. The diagnosis is based on identifying significantly elevated
total erythrocyte protoporphyrin levels, which include a high proportion of
zinc-chelated protoporphyrin. Confirmation of the ALAS2 gene mutation is
achieved through molecular testing. Management focuses on strict sun
protection, supportive care, regular monitoring of liver function, and the use
of pharmacological agents such as Afamelanotide to enhance pain-free light
exposure.
