Sanfilippo
Syndrome, or Mucopolysaccharidosis Type III (MPS III), is a rare autosomal
recessive lysosomal storage disorder caused by deficiencies in one of four
enzymes—heparan‑N‑sulfatase (type A), α‑N‑acetylglucosaminidase (type B),
acetyl‑CoA:α‑glucosaminide acetyltransferase (type C), or N‑acetylglucosamine‑6‑sulfatase
(type D)—all of which lead to impaired degradation and accumulation of heparan
sulfate. The disease primarily affects the central nervous system, with
relatively mild somatic involvement compared to other mucopolysaccharidoses.
Children usually present between ages 2 and 6 with developmental delay,
language regression, hyperactivity, and behavioral disturbances, followed by
progressive cognitive decline. Seizures, motor dysfunction, and gait
abnormalities often emerge in later childhood, and the disease advances to
severe neurodegeneration, loss of mobility, and premature death, typically in
the second or third decade of life. Ocular complications such as retinopathy
may occur, but neurological deterioration is the hallmark of the disorder.
