Pulmonary Alveolar Proteinosis (PAP) Overview

Pulmonary alveolar proteinosis (PAP) is a rare condition characterized by the accumulation of surfactant, consisting of proteins and lipids, within the lung alveoli due to impaired clearance by alveolar macrophages. In primary PAP, the root cause lies in reduced stimulation of alveolar macrophages by GM-CSF, leading to their decreased ability to clear surfactant from the alveoli, resulting in a buildup of surfactant and breathlessness. The clinical presentation of PAP can range from mild to severe, with symptoms often lacking specificity. The most common initial symptom is dyspnea, reported in 39% of patients in one study, followed by a cough, observed in 21% of cases. Hemoptysis, fever, and chest pain are uncommon in autoimmune PAP and should prompt consideration of alternative diagnoses. Fever may occur in 24% of secondary PAP cases, often due to concomitant hematological malignancies or opportunistic infections. Interestingly, a significant proportion, 33%, of patients may be asymptomatic at the time of diagnosis. Most PAP patients have a smoking history (53% to 85%), and many report occupational exposures (39% to 48%). Notably, although 90% of PAP cases are autoimmune, only a small fraction, 1.7%, of patients also have other identified autoimmune diseases. The current standard of care for autoimmune PAP involves a procedure known as whole lung lavage (WLL). The differential diagnosis for pulmonary alveolar proteinosis encompasses conditions with restrictive physiology and diffuse interstitial changes on computed tomography. The disease course of PAP varies, and predicting prognosis can be challenging. However, when autoimmune PAP is treated with whole lung lavage, the 5-year survival rate is an encouraging 95%. In contrast, the prognosis for secondary PAP is less favorable, with a median survival time of less than 15 months. Common causes of death in secondary PAP patients include underlying hematological disease (33%), infection (25%), respiratory failure (25%), and bleeding complications (13%). Patients with PAP face an elevated risk of developing opportunistic infections, with approximately 5% of PAP patients experiencing such infections. While lung infections are the most common, extra-pulmonary infections comprise 32% of opportunistic infections in PAP patients.
