Neurotrophic
Keratitis (NK) is a rare, degenerative corneal disease caused by damage to the
trigeminal nerve, which innervates the cornea. This neural damage impairs both
the sensory function (leading to reduced or absent corneal sensation) and the
trophic function (which provides essential neurotrophic support for corneal
health and wound healing). The resulting loss of sensation and impaired healing
leads to a progressive breakdown of the corneal epithelium, ulceration, and,
potentially, stromal melting and perforation, which can severely threaten
vision. Common causes of NK include viral infections (like Herpes Simplex and
Herpes Zoster), diabetes mellitus, ocular or neurosurgical trauma, and
long-term use of certain topical medications. The disease is often insidious
because the loss of sensation means patients rarely report pain. Symptoms,
which can be mild, include blurred vision, redness, and dry eye sensation. NK
is clinically staged by severity: Stage 1 involves punctate epithelial erosions
and epithelial irregularity; Stage 2 is characterized by a persistent
epithelial defect (PED); and Stage 3 involves a corneal ulcer and the risk of
perforation.
