Cocaine
intoxication and abuse are characterized by adrenergic hyperactivity and
increased blood pressure. Cocaine use is linked to acute hypertension but not

40-year-old woman was treated successfully with nitroprusside and captopril for
hypertensive encephalopathy caused by cocaine abuse. Cocaine use during
pregnancy increases the risk of premature placental abruption and blood
pressure elevation, which is less responsive to standard therapy than
pregnancy-induced hypertension. Cocaine use has been linked to sudden death due

epilepticus, and impaired thermoregulation. Cocaine use during pregnancy
increases the risk of placental abruption and emergency cesarean delivery. This
medication binds and blocks monoamine reuptake transporters (dopamine,
norepinephrine, and serotonin) with equal affinity. Monoamines accumulate in
the synaptic cleft, causing sympathetic effects to be enhanced and prolonged.
Cocaine’s primary effects on the cardiovascular system are due to alpha- and
beta-1-adrenoceptor stimulation, which causes an increase in heart rate, systemic
arterial pressure, and myocardial contractility, all of which are important
determinants of myocardial oxygen demand. Cocaine and its metabolites can cause
arterial vasoconstriction for several hours after use.
