Subsequently, the progression from MBL to CLL/SLL occurs further due to the B-cell clone, driven by additional genetic aberrations or alterations in the bone marrow microenvironment. Notably, the expression of B-cell antigen receptors (BCR) instigates antigen-independent, cell-autonomous signaling, playing a pivotal role in CLL pathogenesis. Fluorescence in situ hybridization (FISH) is a compassionate diagnostic tool for detecting chromosomal abnormalities in CLL/SLL patients. Some individuals may manifest with SLL involving solitary lymph node sites. In younger patients capable of tolerating chemotherapy and lacking 17p deletion/TP53 mutation but exhibiting IGHV mutated CLL, fludarabine, cyclophosphamide, and rituximab (FCR) constitute an initial treatment option. The primary management approach for localized SLL mirrors that of early-stage asymptomatic CLL, predominantly relying on observation unless there’s an evident “active disease,” warranting radiation therapy (RT).
