Choroideremia Overview

Choroideremia is a rare X-linked recessive genetic disorder characterized by progressive vision loss, primarily affecting males. It is caused by mutations in the CHM gene located at Xq21.2, responsible for producing the Rab escort protein-1 (REP-1). In affected males, the condition typically manifests as night blindness (nyctalopia) during their first or second decade. This is followed by a narrowing of the peripheral visual field, with a progression from ring-shaped blind spots (annular scotomas) to a more extensive visual field loss. Visual acuity impairment becomes noticeable by mid-adulthood. Over time, changes in the fundus (the back of the eye) are observed, initially showing pigmentary stippling and focal areas of choroid atrophy in the equatorial fundus. In the advanced stages, extensive degenerative changes occur in the retinal pigment epithelium (RPE), with only remnants of the choroidal blood vessels remaining apparent in the macula, far peripheral retina, and near the optic disc. At this stage, the sclera becomes visible during fundus examination in areas where the choroidal and RPE atrophy is complete. Managing Choroideremia involves regular ophthalmologic examinations to monitor the condition’s progression or the emergence of cataracts. Additionally, individuals are advised to use UV-blocking sunglasses to protect their eyes. Unfortunately, there is currently no available treatment for Choroideremia.
