Acute Coronary Syndrome (ACS) Overview

Acute coronary syndrome (ACS) encompasses a spectrum of coronary artery diseases, including unstable angina, ST-elevation myocardial infarction, and non-STEMI. Differentiating acute coronary syndrome from noncardiac chest pain is the primary diagnostic challenge. Symptoms of the acute coronary syndrome include chest pain, referred pain, nausea, vomiting, dyspnea, diaphoresis, and lightheadedness. In one study, sudden dyspnea was the only presenting feature in 4 to 14 percent of patients with acute myocardial infarction. The arm, the jaw, the neck, the back, or even the abdomen can all be referred to as painful. Acute coronary syndrome is more likely if the pain radiates to the shoulder, left arm, or both arms. Typical angina is described as substernal pain that occurs on exertion and is relieved with rest. Patients with all three of these characteristics are more likely to have acute coronary syndrome than patients who have none, one, or even two. Chest pain that occurs suddenly at rest or in a young patient may indicate Prinzmetal’s angina or acute coronary vasospasm, which occurs with cocaine or methamphetamine. The acute coronary syndrome affects only about 2% of patients with cocaine-related chest pain.
