Activated
PI3K Delta Syndrome (APDS) Overview
Activated
PI3K Delta Syndrome (APDS), also known as PASLI disease, is a combined
immunodeficiency characterized by dominant, gain-of-function mutations in
PIK3CD and PIK3R1 genes, encoding the catalytic and regulatory subunits of
PI3Kδ. This leads to hyperactivation of the PI3K/AKT/mTOR/S6K
pathways in immune cells. Manifestations include recurrent respiratory tract
infections, bronchiectasis, herpesvirus infections, autoimmunity,
lymphoproliferation, lymphoma, neurodevelopmental delay, and growth retardation.
APDS1 and APDS2 differentiate mutations in PIK3CD and PIK3R1, respectively. No
curative therapy exists, but management strategies like antibiotic prophylaxis
and immunoglobulin replacement therapy are available. HSCT has shown efficacy,
particularly in young patients.
