Overview
Achromatopsia
is a rare autosomal recessive retinal disorder that leads to color blindness,
nystagmus, photophobia, and severely reduced visual acuity due to cone function
impairment or absence. It is characterized by reduced visual acuity, pendular
nystagmus, photophobia, a small central scotoma, and reduced or complete loss
of color discrimination. Most individuals with ACHM exhibit complete ACHM,
where all three types of cones lack function, resulting in severe symptoms. In
some rare cases, incomplete ACHM occurs, with milder symptoms due to the
presence of functional cones. ACHM is associated with five genes: GNAT2, PDE6C,
PDE6H, CNGA3, and CNGB3, all critical components of the cone phototransduction
cascade. CNGB3 mutations are the most common, followed by CNGA3, while others
are rare causes of ACHM. Achromatopsia comes in two types: complete, with no
functional cones, and incomplete, with some functional cones. The differential
diagnosis includes conditions like blue cone monochromatism, Leber congenital
amaurosis, other cone dystrophies, and cerebral achromatopsia.
