Vulvar Cancer Overview
Vulvar
cancer, also known as vulval cancer or cancer of the vulva, originates in the
tissues of a woman’s external genital area. It is relatively uncommon and can
manifest in various parts of the vulva, including the labia majora and minora,
clitoris, vaginal opening, and perineum (the area between the vulva and anus).
The most prevalent type of vulvar cancer is squamous cell carcinoma (SCC),
which develops from the squamous cells lining the vulvar surface. Other less
common types include adenocarcinoma, melanoma, and basal cell carcinoma.
Typical symptoms of vulvar cancer may include itching, burning, soreness, or
pain in the vulva, the presence of a lump, sore, swelling, or wart-like growth,
changes in the shape or color of a mole on the vulva, discharge from a lesion
or sore spot in the vulva, and swollen lymph nodes in the groin area. Several
risk factors contribute to vulvar cancer, including increasing age, human
papillomavirus (HPV) infection, smoking, inflammatory vulvar conditions, prior
pelvic radiation, and immunodeficiency. Diagnosing vulvar cancer can be
challenging due to its sometimes nonspecific nature, and other conditions like
cutaneous SCC, cutaneous BCC, cutaneous melanoma, and various skin disorders
may mimic its symptoms. Surgical excision is the standard treatment for vulvar
cancer, while adjuvant radiation and chemotherapy may be recommended based on
the histopathology and extent of the disease. Lymphatic spread in vulvar cancer
typically involves homolateral superficial inguinal lymph nodes and, later, the
deep femoroinguinal lymph nodes. Lymph node status significantly influences
overall survival, with lower survival rates observed in patients with lymph
node metastasis. Adjuvant radiotherapy can improve progression-free survival in
node-positive patients. The recurrence rate of vulvar cancer is 37% at five
years, and distant metastasis leads to a poor prognosis. Complications of
vulvar cancer may include increased pain and irritation as the malignancy
spreads and invades surrounding tissues. Surgical complications are common,
such as infection, blood loss, lymphedema in the lower extremities, chronic
pain, and organ-related issues during additional procedures. Chemotherapy and
radiation can lead to susceptibility to disease, gastrointestinal reactions,
tissue fibrosis, and lymphedema.
