Primary
IgA nephropathy (IgAN) is the most prevalent form of primary glomerulonephritis
globally and a significant contributor to chronic kidney disease and end-stage
renal disease. The disease, also known as Berger’s disease, presents a diverse
clinical and pathological course, often considered a spectrum of diseases
rather than a single entity. While the exact cause remains unknown, it is
thought to be an autoimmune process initiated by the production of an abnormal
IgA1 protein that deposits in the kidneys’ filtering units, the glomeruli. This
deposition leads to inflammation and damage, impairing the kidneys’ ability to
filter waste. Clinical presentation is highly variable and can range from
asymptomatic microscopic hematuria to episodes of visible blood in the urine,
often following an upper respiratory infection. Some patients may also present
with significant proteinuria, with or without nephrotic syndrome. The only way
to definitively diagnose primary IgAN is through a kidney biopsy. Risk stratification
is crucial for management and is based on a combination of clinical,
laboratory, and pathological data. Low-risk patients have minor urinary
abnormalities and normal kidney function, while higher-risk patients are
defined by persistent proteinuria and/or a decline in their estimated
glomerular filtration rate (eGFR). Given the heterogeneity of the disease,
management strategies are highly individualized, ranging from supportive care
to targeted immunomodulatory therapies.
