Inflammatory Disease (PID) is an inflammatory condition affecting the upper
genital tract in women, primarily involving the uterus, fallopian tubes, and
ovaries. It is predominantly caused by an ascending infection, wherein
pathogenic bacteria from the lower genital tract ascend to the upper tract.
Among PID cases, approximately 85% are attributed to sexually transmitted
bacteria. The most commonly implicated pathogens responsible for these
infections are Neisseria gonorrhoeae and Chlamydia trachomatis. PID resulting
from N. gonorrhoeae tends to be more severe than other causes. On the other
hand, PID caused by chlamydia may often remain asymptomatic, leading to
subclinical PID. Subclinical PID is concerning as it lacks apparent symptoms
but can still have significant long-term repercussions. Infection in the upper
genital tract leads to inflammation and subsequent damage, resulting in
scarring, adhesions, and potential obstruction of the Fallopian tubes. The loss
of ciliated epithelial cells along the fallopian tube lining due to scarring
impairs ovum transport, leading to an increased risk of infertility and ectopic
pregnancy. Furthermore, the presence of adhesions can contribute to chronic
pelvic pain. The recommended first-line outpatient treatment for PID, according
to CDC guidelines, involves a combination of doxycycline (100 mg orally twice a
day for 2 weeks) and either ceftriaxone 500 mg intramuscularly for a single
dose or cefoxitin 2 g intramuscularly with probenecid 1g orally for a single dose.
Alternatively, another parenteral third-generation cephalosporin may be used.
When considering PID as a possible diagnosis, it is essential to differentiate
it from other conditions such as ectopic pregnancy, cystitis, ovarian torsion,
endometriosis, ovarian cyst rupture, appendicitis, diverticulitis, traumatic
injury, and pyelonephritis.
