Chronic Otitis Media (COM) Overview
Chronic
otitis media (COM) is characterized by the chronic presence of middle ear
effusion in the absence of acute signs of infection or chronic complications
from otitis media, including persistent tympanic membrane perforation.
Granulation tissue may be soft or fibrous, contain cholesterol or hemorrhage,
and coexist with cholesteatoma. Chronic otitis media is divided into chronic
suppurative otitis media and chronic otitis media with effusion (OME). Chronic
suppurative otitis media is not temporally related to acute otitis, as the
pathological changes in the middle ear are different. The classic symptoms of
established chronic suppurative otitis are otorrhea and deafness or some form
of hearing loss. Chronic suppurative otitis media can lead to the formation of
cholesteatoma. This cyst contains desquamated epithelial cells, which can cause
local bone erosion and lead to the retention of infected material.
