Cholera Overview
Cholera is
an acute diarrheal illness caused by infection with the bacterium Vibrio
cholera, specifically the O1 and O139 serogroups. Upon ingestion, V. cholera is
killed by gastric acid, but surviving organisms colonize the small intestine
and produce cholera toxin. Transmission occurs through contaminated food and
water; risk factors include drinking untreated water and consuming certain
foods, such as shellfish. Other risk factors include blood group O,
hypochlorhydria, and partial gastrectomy. The ingestion of V. cholerae can
result in the colonization of the small intestine. Utilizing its flagella, the
organism navigates through the mucus layer and reaches the intestinal wall.
Once there, toxigenic V. cholerae secretes toxin-coregulated pilus, facilitating
attachment to ganglioside receptors in the mucosal barrier. This attachment
prompts the production of cholera toxin, which ADP-ribosylates the Gs subunit
of the G protein complex within the gut epithelium.
