Monkeypox is a zoonotic disease historically endemic to West and Central Africa, caused by the monkeypox virus (MPXV). This virus belongs to the Orthopoxvirus (OPV) genus within the Poxviridae family, which also includes the variola virus (VARV, or smallpox), vaccinia virus (VACV), camelpox virus (CMPV), and cowpox virus (CPXV), all of which are pathogenic to humans. The monkeypox virus is characterized by double-stranded DNA (dsDNA) and exhibits a brick-like morphology with particles measuring 200–250 nm in diameter. Two genetic clades of MPXV have been identified: the West African (WA, Clade II) clade and the Congo Basin (CB, Clade I) clade, the latter also known as the Central African (CA) clade, which is associated with more severe disease. Polymerase chain reaction (PCR) is the preferred method for detecting MPXV DNA. When feasible, optimal diagnostic specimens are collected from the rash, including skin, fluid, crusts, or biopsy samples. Antigen and antibody detection methods may be less useful as they must differentiate between orthopoxviruses. There is limited comprehensive data on the pathophysiology and treatment of monkeypox.
