Radioactive iodine I-131 (RAI) treatment is the primary systemic therapy for advanced disease (5). RAI refractoriness is most commonly associated with the loss of thyroid differentiation features. A clear indicator of dedifferentiation is the dysfunction of the Na/I symporter (NIS). Standard protocols for identifying recurrent and metastatic DTC following total/near total thyroidectomy and RAI therapy involve serum thyroglobulin (Tg) tests in conjunction with RAI whole body scans (WBS).
