Neisseria gonorrhea is an obligate human pathogen primarily transmitted through sexual contact. It infects humans exclusively and commonly causes urethritis in men and cervicitis in women. As an obligate pathogen, N. gonorrhea relies on manifesting disease to facilitate transmission between hosts. It cannot survive outside of a host and must infect a host. The infection process of N. gonorrhea begins with the adherence of gonococci to epithelial cells, followed by local invasion of these cells. Adhesion is facilitated by multiple surface proteins, including pili, hair-like appendages covering the bacterial surface. Pili allow the bacteria to attach to epithelial cells from a distance and promote cellular invasion. Additionally, pili provide motility and protection to the bacteria. Other surface proteins involved in cellular attachment include Opa (opacity-associated proteins) and LOS (lipooligosaccharide), which can attach to sperm cells and potentially contribute to transmission from infected males to uninfected sexual partners. Symptoms of gonorrhea may include green or yellow discharge from the penis, pain or burning sensation during urination, discomfort and swelling of the testicles (in men), increased vaginal discharge, lower abdominal pain (particularly during sex), and bleeding between periods or after sex. In routine settings, the diagnosis of gonorrhea can be verified quickly through direct microscopy of urethral or cervical smears stained with Gram or methylene blue. Gram staining is more specific than methylene blue staining. Risk factors for N. gonorrhea infection include sexual contact with new or multiple partners, sexual contact with individuals who have concurrent partners or current N. gonorrhea infection, inconsistent condom use with nonmonogamous partners, previous history of N. gonorrhea infection, and engaging in sex for money, drugs, or other items. Gonorrhea has increased over the past 20 years due to antimicrobial resistance. If gonorrhea infections are not detected or appropriately treated, they can lead to serious reproductive health complications in women, such as pelvic inflammatory disease, chronic pelvic pain, infertility, first-trimester abortion, and ectopic pregnancy.
- In the USA, the highest reported cases of gonorrhea were observed in women aged 20 to 24 years (866.9 per 100,000), followed by those aged 15 to 19 years (616.4 per 100,000) and 25 to 29 years (505.3 per 100,000). Among men, the highest reported cases were found in those aged 20 to 24 years (821.5 per 100,000), followed by those aged 25 to 29 years (727.9 per 100,000) and 30 to 34 years (567.6 per 100,000).
