Lichen planus is a chronic autoimmune disease impacting the skin, oral and genital mucosa, scalp, and nails. Its lesions are characterized by six features known as the six P's: flat-topped, purple, polygonal, pruritic, papules, and plaques. These papules and plaques, which are itchy and violet in color, are typically found on the wrists, lower back, and ankles. Wickham striae, a lattice-like network of white lines, can be observed, especially on the buccal mucosa, which may also exhibit erosions. Drug-induced lichen planus, or lichenoid drug eruption, often presents similarly and is mainly distributed due to photosensitivity, making it hard to distinguish from the idiopathic form. The exact cause of lichen planus is unknown. However, it is believed to be a T-cell-mediated autoimmune disease triggered by various external factors such as viruses, drugs, or allergens. These factors lead to changes in self-antigens on the skin and activation of cytotoxic CD8+ T cells, causing T-cell targeting and apoptosis of basal keratinocytes. Several conditions, including lupus erythematosus (LE), lichen nitidus, psoriasis, secondary syphilis, pityriasis rosea, erythema dyschromicum perstans, graft versus host disease, and keratosis lichenoides chronica, may mimic lichen planus. Differentiating between LP and LE can be challenging, especially when they affect the scalp or mouth. A biopsy with direct immunofluorescence (DIF) is valuable in such cases. Co-occurrence of both diseases has been reported and may be linked to antimalarial treatments for LE. Cutaneous lichen planus often resolves on its own within one to two years, but hyperpigmentation can persist. Oral lichen planus may clear spontaneously within five years but frequently has a chronic, recurring nature. Hair loss from lichen planopilaris is permanent. Withdrawal of the causative medication is the primary treatment for drug-induced lichen planus. Topical corticosteroids are the initial treatment for mucosal erosive lichen planus, while systemic corticosteroids are an option for severe, widespread cases affecting the mouth, skin, or genitals. Dermatologist referral for systemic therapy with expensive and potentially toxic oral retinoids or immunosuppressants may be necessary for severe, treatment-resistant lichen planus. Oral involvement occurs in approximately 50% of cases and may occur independently of skin lesions. Vulvar and vaginal mucosae are frequently affected, and undiagnosed vulvar lichen planus is common in women with oral mucosal symptoms. Genital involvement, particularly of the glans penis, is frequent in men, and nail involvement is seen in up to 10% of cases. The average age at diagnosis is 62.4 years.
