Inflammatory bowel disease (IBD) is a chronic inflammatory condition affecting the gastrointestinal tract. It arises due to an abnormal immune response to gut microflora, resulting in recurrent episodes of inflammation. The two primary forms of IBD are Crohn's disease and ulcerative colitis (UC). IBD develops in genetically susceptible individuals when their immune system reacts inappropriately to the intestinal flora. Crohn's disease is characterized by segmental inflammation that affects the entire GI tract, often with granulomas. In contrast, UC primarily involves inflammation of the colon and rectal mucosa. These conditions are relatively common in pediatric patients, with a significant number diagnosed before age 20. Inflammatory bowel disease can also extend to the tracheobronchial tree, affecting the larynx, trachea, and both large and small airways, leading to airway inflammation. Large airway inflammation is the most frequent, occurring in approximately 40% of IBD patients. About 10% of patients may experience upper respiratory tract or small airway involvement. The primary distinction between Crohn's disease and ulcerative colitis lies in their anatomical distribution. Crohn's disease can impact any part of the gastrointestinal tract, although it is most common in the ileum and colon. It displays a segmental, non-continuous inflammation, often with transmural involvement. Complications such as deep-penetrating ulcerations, fistulas, and abscesses can occur. A distinctive histological feature is the presence of epithelioid granulomas. On the other hand, UC is confined to the colon and rectum, featuring continuous and diffuse mucosal inflammation that typically begins distally and progresses proximally.
