Complex
regional pain syndrome (CRPS), or Sudeck's dystrophy or reflex sympathetic
dystrophy, is a painful condition characterized by various symptoms, including
pain, sensory disturbances, Sudo- and vasomotor abnormalities, trophic changes,
and impaired motor function. CRPS can develop due to different levels or types
of tissue damage. In some cases, it has even been observed without any injury,
often arising from extended periods of immobilization. Among the most common
causes of CRPS is a fracture, while surgery is another frequent trigger. Other
injuries or insults that can incite CRPS encompass sprains, contusions, crush
injuries, and surgical procedures. CRPS is categorized into two main types:
type I, formerly known as reflex sympathetic dystrophy, and type II, previously
termed causalgia. Type I occurs without associated nerve trauma, whereas type
II arises following known nerve injuries. Additionally, CRPS is further
subdivided into "warm" versus "cold" varieties, and
sympathetically-maintained (SMP) versus sympathetically-independent (SIP)
forms, which can influence both the prognosis and available treatment options.
Patients with CRPS receive treatment from a diverse range of medical
specialists, including anaesthesiologists, (orthopaedic) surgeons,
neurologists, rheumatologists, and rehabilitation centers, due to the
multifaceted nature of the condition and its wide array of symptoms. It's
important to note that there are currently no medications approved by the Food
and Drug Administration (FDA) specifically for treating CRPS. Consequently,
treatment protocols frequently require the use of off-label medications.
Limitations in available medications could potentially restrict options for
both healthcare providers and patients in managing their symptoms. The
prognosis of CRPS varies, with instances of spontaneous remission and
refractory clinical presentations observed. However, early intervention may
lead to improved outcomes. Long-standing CRPS can be associated with
complications such as dystonia, cognitive executive dysfunction, adrenal
insufficiency, gastroparesis, and irritable bowel syndrome.
