Chronic otitis media (COM) is characterized by the chronic presence of middle ear effusion in the absence of acute signs of infection or chronic complications from otitis media, including persistent tympanic membrane perforation. Granulation tissue may be soft or fibrous, contain cholesterol or hemorrhage, and coexist with cholesteatoma. Chronic otitis media is divided into chronic suppurative otitis media and chronic otitis media with effusion (OME). Chronic suppurative otitis media is not temporally related to acute otitis, as the pathological changes in the middle ear are different. The classic symptoms of established chronic suppurative otitis are otorrhea and deafness or some form of hearing loss. Chronic suppurative otitis media can lead to the formation of cholesteatoma. This cyst contains desquamated epithelial cells, which can cause local bone erosion and lead to the retention of infected material. Occasionally, the patient may have more advanced chronic ear pathology symptoms, such as pain, vertigo, or facial nerve paralysis. OME is one of the most common diseases in childhood. It is related to infection, eustachian tube obstruction, allergic or immunological disorders, and enlarged adenoids. The serous fluid in chronic suppurative otitis media contains bacteria such as H. influenzae and pneumococcus and is predisposed to developing OME. OME has been implicated as an immune-mediated disease, as immune complexes have been demonstrated in middle ear effusion and highly organized lymphatic tissue has been found in the middle ear mucosa. Surgery is recommended when medical treatment fails, especially when the child has hearing loss. Tympanostomy tubes appear beneficial in OME but are of less value in chronic suppurative otitis.
