AL amyloidosis, also known as amyloid light chain amyloidosis, is a rare condition where a small B-cell clone, often a plasma cell clone, produces monoclonal light chains. These light chains can cause organ toxicity and deposit in tissues as amyloid fibrils. The clinical manifestations depend on the affected organs, but symptoms are often recognized late. Patients diagnosed at advanced stages, especially when heart involvement is present, face a high risk of death within a few months. Autologous Stem Cell Transplantation is an option for approximately one-fifth of patients upfront or after bortezomib-based conditioning. Immune-modulatory drugs are commonly used for relapsed/refractory disease. Daratumumab is an effective option for relapsed/refractory cases. Patients who reach excellent partial response (VGPR) may tolerate a gradual rise in free light chains (dFLC), assuming their dFLC was not <50 mg/L at diagnosis. Second-line therapy for relapsed/refractory AL amyloidosis (RRAL) may include daratumumab or daratumumab-based therapy.
