Partial seizures, often referred to as
"focal seizures," primarily impact a specific brain region, with the
temporal lobes being the most common location of occurrence. They can be
categorized into two main types: simple partial seizures, which occur without a
loss of consciousness, and complex partial seizures, which involve impaired
consciousness. When a partial seizure extends to affect the entire brain, it is
termed a "partial seizure with secondary generalization." Partial
seizures are the most prevalent form of seizures among individuals with
epilepsy. Notably, complex partial seizures are more frequent than simple
partial seizures, though many complex partial seizures initially manifest as
simple partial seizures. Seizures result from episodes of excessive and
disorganized neuronal activity within the brain. Ordinarily, electrical signals
are conveyed through an action potential with a net influx of positive ions,
leading to neuron depolarization. Hyperexcitability is typically held in check
by various inhibitory mechanisms involving negative ions like chloride ions and
inhibitory neurotransmitters. Altered voltage-gated ion channels can disrupt
this balance, leading to heightened excitatory transmission and reduced
inhibitory signals, thereby causing hyperexcitability. Glutamate serves as the
primary excitatory neurotransmitter in the brain, while GABA acts as the
inhibitory counterpart. When excitation prevails over inhibition in neurons,
hyperexcitability occurs. The convergence of an epileptic focus,
hyperexcitability, and hyper-synchronization among neurons ultimately triggers
seizures. There is no drug of choice for treating seizures. Commonly employed
first-line medications for partial epilepsy encompass carbamazepine and
lamotrigine. Secondary options may be considered if these medications fail to
control seizures, including valproate, topiramate, oxcarbazepine, or
gabapentin. Notably, lamotrigine is the preferred treatment for partial
seizures associated with secondary generalization. Patients experiencing
partial seizures face a greater risk of seizure recurrence than those with
generalized seizures. However, recurrence rates appear similar between simple
and complex partial seizures. Furthermore, simple partial seizures, which do
not involve a loss of consciousness, carry a lower risk of seizure-related
complications such as aspiration and exhibit a reduced overall morbidity and
mortality rate compared to seizures that impair consciousness. Simple partial
seizures can also result in falls and injuries.

the CDC, approximately 3.4 million people in the United States are affected by
