 Hand eczema, or dermatitis, is frequently a persistent and complex condition. Its primary indications encompass dry, itchy skin that appears red or darker than the surrounding skin. Certain professions, including hairdressers, farmers, construction workers, dental and medical personnel, metalworkers, and food handlers, are prone to developing hand eczema. Various clinical patterns of hand eczema exist, primarily differing in clinical presentation rather than histology. These patterns include hyperkeratotic (resembling psoriasis or tylotic eczema), frictional, nummular, atopic, pompholyx (also known as dyshidrosis), and chronic vesicular hand dermatitis. Hybrid variations of these patterns are also observed, and there is some disagreement among experts regarding their classification. Clinically, hand eczema is characterized by observable signs such as redness, vesicles, papules, scaling, fissures, hyperkeratosis, and bothersome symptoms like itching and pain. "chronic hand eczema" applies when the condition persists for over six months. While diagnosing hand eczema may seem straightforward, it can occasionally be challenging to distinguish it from other dermatological conditions such as psoriasis, tinea mannum, lichen planus, pityriasis rubra pilaris, and palmar pustulosis. Treatment for hand eczema traditionally involves topical corticosteroids, oral antihistamines, emollients, and, in severe cases, short courses of systemic corticosteroids based on the disease's severity. Topical steroids are typically the first-line treatment for managing inflammation, with various types like desonide, mometasone furoate, clobetasol propionate, and betamethasone dipropionate, among others, being employed in hand eczema therapy.
