Malignant mesothelioma is a rare form of cancer arising from the mesothelial cells that line various viscera. It is primarily associated with asbestos exposure and is observed in the pleural lining (malignant pleural mesothelioma). The exact mechanisms by which asbestos exposure leads to the development of malignant mesothelioma are not fully understood but are believed to involve repeated pleural inflammation, interference with cell division, activation of proto-oncogenes, and oxidative stress. The three histological subtypes of mesothelioma are epithelioid, sarcomatoid, and mixed, with the epithelioid subtype associated with a better prognosis. Malignant pleural mesothelioma spreads locally, initially affecting the parietal pleura and then spreading to the visceral pleura, chest wall, diaphragm, or mediastinum. Direct invasion of regional lymph nodes, particularly the bronchopulmonary or hilar nodes, is also observed. The initial symptoms of malignant pleural mesothelioma are usually vague and atypical, including abdominal pain, distension, weight loss, anorexia, and ascites. The differential diagnosis for this condition includes several other lung cancers, benign pleural masses, and lung infections. Multiple factors, including the presentation stage, gender, and the extent of surgical resection, influence the prognosis of malignant pleural mesothelioma. Patients who underwent successful surgical resection have a significantly better prognosis, with longer median survival and higher 5-year survival rates.
