Diffuse
large B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin lymphoma
(NHL. It represents a heterogeneous malignancy concerning molecular
alterations, morphology, clinical behavior, and treatment response. Due to the
aggressive character of DLBCL, this lymphoid malignancy proliferates and tends
to involve both nodal and extranodal sites, including the stomach, testis, and
CNS; Relapse occurs in approximately 25–30% of DLBCL patients, with 10%
exhibiting refractory disease. The majority of DLBCL relapses occur within two
years of diagnosis. However, relapses after five years still happen in a
fraction of DLBCL patients. They are linked to a favorable International
Prognostic Index (IPI) score, limited-stage disease, and extranodal involvement
at diagnosis. Survival rates have enhanced over the last several decades, with
the most recent 5-year relative survival rate reported as 62.0% in the United
States and 55.4% in Europe. More than 50% of patients with advanced-stage de
novo DLBCL are cured with rituximab combined with cyclophosphamide,
doxorubicin, vincristine, and prednisone (R-CHOP). However, depending on the number
of adverse prognostic factors from the International Prognostic Score (IPI),
20% to 50% of patients with DLBCL will be refractory to R-CHOP or relapse after
achieving complete response (CR).
Approximately
50 to 60% of patients with DLBCL achieve and maintain complete remission after
first‑line therapy; 30 to 40% relapse and 10%
have refractory disease.
DEL is
more frequent among R/R DLBCL (45%) than primary DLBCL (20–35%) and is
associated with inferior outcomes. Furthermore, DHITsig-positive cases are more
common (60 percent) in diagnosis samples of relapsed DLBCL patients than
non-relapsing patients (30 percent ).
